Your Rights
- Access your records: You have the right to review or request copies (electronic or paper) of your medical records and any health information we maintain. Our team can guide you through the process. Access or summaries are generally provided within 30 days, and reasonable fees may apply.
- Request Amendments: If your records contain incorrect or incomplete information, you may request corrections. We will respond in writing and may deny requests under specific circumstances, providing an explanation.
Request Confidential Communications
- You may ask us to communicate with you in a specific way (for example, by phone, email, or mail) or to send correspondence to a designated address. We will accommodate all reasonable requests to ensure your privacy is protected.
- Request Restrictions on Sharing: You can request that certain health information not be used or disclosed for treatment, billing, or administrative purposes.
- While we will consider your request, we are not obligated to comply if it would affect your care.
- If you pay out-of-pocket for services in full, you may request that we do not share that information with your insurer. We will honor such requests unless a law requires disclosure.
Access a Record of Disclosures
- You have the right to request a list of times we have shared your health information in the past six years, including the recipients and reasons.
- This excludes routine disclosures for treatment, payment, or operations. One accounting is provided free every 12 months; additional requests may incur a reasonable fee.

- You can request a printed copy of this HIPAA notice at any time, even if you previously received it electronically.
Appoint Someone to Act on Your Behalf
- If you have a legal guardian or someone with medical power of attorney, they may exercise your rights on your behalf.
- We will verify their authority before taking action.
Register a Complaint if Your Rights Are Violated

- Sale of PHI
- Most psychotherapy notes
For Fundraising Purposes:
- We might reach out to you for fundraising efforts, but you have the right to opt out of future communications.
- Your health information may also be shared with other healthcare professionals involved in your treatment.
